How it works
Vildagliptin works by inhibiting DPP-4 activity, which increases the levels of incretin hormones (GLP-1 and GIP). These hormones help regulate blood sugar by increasing insulin release and decreasing glucagon levels. Metformin HCl reduces glucose production in the liver, decreases intestinal absorption of glucose, and improves insulin sensitivity, helping the body use glucose more effectively.
Usage and Dosage
The dose depends on the patient’s current metformin intake. It can be started as 50mg + 500mg, 50mg + 850mg, or 50mg + 1000mg twice daily (morning and evening). It should be taken with meals to reduce stomach-related side effects. The total daily dose of vildagliptin should not exceed 100 mg. Not recommended for children.
Indication
This combination is used to treat Type 2 Diabetes Mellitus. It is prescribed along with diet and exercise when blood sugar is not well controlled with metformin alone or when both medicines are already being taken separately.
Side Effects
Common side effects include hypoglycemia, headache, dizziness, tremor, nausea, fatigue, sweating, diarrhea, flatulence, chills, and gastro-esophageal reflux.
Drug Interactions
It may interact with thiazides, corticosteroids, thyroid products, sympathomimetics, furosemide, nifedipine, and certain cationic drugs such as digoxin, morphine, quinidine, and vancomycin. Other interacting substances include estrogens, oral contraceptives, phenytoin, nicotinic acid, and alcohol.
When not to use
Do not use in patients with hypersensitivity to vildagliptin or metformin, or in cases of diabetic ketoacidosis or diabetic pre-coma.
Precautions
This combination is not a substitute for insulin and should not be used in patients with Type 1 diabetes.
Warnings
Monitor for skin problems like blistering or ulcers. There is a risk of acute pancreatitis; discontinue use if suspected and do not restart if confirmed. Use caution in patients with a history of pancreatitis. Metformin should be stopped 48 hours before surgery with general anesthesia and restarted only after 48 hours.
