How it works
Moxifloxacin has in vitro activity against a wide range of Gram-positive and Gram-negative microorganisms. The bactericidal action of moxifloxacin results from inhibition of the topoisomerase II (DNA gyrase) and topoisomerase IV required for bacterial DNA replication, transcription, repair, and recombination. It appears that the C8-methoxy moiety contributes to enhanced activity and lower selection of resistant mutants of Gram-positive bacteria compared to the C8-H moiety. The presence of the bulky bicycloamine substituent at the C 7 position prevents active efflux, associated with the NorA or pmrA genes seen in certain Gram-positive bacteria.

Indication
It is indicated for the treatment of adults (> 18 years of age) with infections caused by susceptible strains of the designated microorganisms in the conditions listed below:Acute Bacterial Sinusitis caused by Streptococcus pneumoniae, Haemophilus influenzae, or Moraxella catarrhalis. Acute Bacterial Exacerbation of Chronic Bronchitis caused by Streptococcus pneumoniae, Haemophilus influenzae, Haemophilus parainfluenzae, Klebsiella pneumoniae, methicillin-susceptible Staphylococcus aureus, or Moraxella catarrhalis.Community Acquired Pneumonia caused by Streptococcus pneumoniae.
When not to Use
Moxifloxacin is contraindicated in persons with a history of hypersensitivity to moxifloxacin or any member of the quinolone class of antimicrobial agents.

Warning 1
Moxifloxacin has been shown to prolong the QT interval of the electrocardiogram in some patients. The drug should be avoided in patients with known prolongation of the QT interval, patients with uncorrected hypokalemia and patients receiving Class IA (e.g., quinidine, procainamide) or Class III (e.g., amiodarone, sotalol) antiarrhythmic agents, due to the lack of clinical experience with the drug in these patient populations.
Warning 2
Moxifloxacin should be used with caution in patients with ongoing proarrhythmic conditions, such as clinically significant bradycardia, acute myocardial ischemia. The magnitude of QT prolongation may increase with increasing concentrations of the drug or increasing rates of infusion of the intravenous formulation. Therefore the recommended dose or infusion rate should not be exceeded.
Warning 3
Convulsions have been reported in patients receiving quinolones. Quinolones may also cause central nervous system (CNS) events including: dizziness, confusion, tremors, hallucinations, depression and rarely, suicidal thoughts or acts. These reactions may occur following the first dose. If these reactions occur in patients receiving moxifloxacin, the drug should be discontinued and appropriate measures instituted.

Bosentan is a dual endothelin receptor antagonist with affinity for both ETA and ETB receptors. Bosentan decreases both pulmonary and systemic vascular resistance, resulting in increased cardiac output without increasing heart rate. The efficacy of bosentan has not been established in patients with severe Pulmonary arterial hypertension (PAH). Transfer to a therapy that is recommended at the severe stage of the disease (e.g. epoprostenol) should be considered if the clinical condition deteriorates .

Bosentan should only be initiated if the systemic systolic blood pressure is higher than 85 mmHg.
Warning 3
Bosentan has not been shown to have a beneficial effect on the healing of existing digital ulcers.

How it works

Warning 1
Patients should be followed carefully for any signs of bleeding including occult bleeding, especially during the first weeks of treatment and/or after invasive cardiac procedures or surgery. The concomitant administration of Clopidogrel/Aspirin with oral anticoagulants is not recommended since it may increase the intensity of bleeding.
Warning 2
Acquired haemophilia has been reported following use of clopidogrel. In cases of confirmed isolated activated Partial Thromboplastin Time (aPTT) prolongation with or without bleeding, acquired haemophilia should be considered. Patients with a confirmed diagnosis of acquired haemophilia should be managed and treated by specialists, and clopidogrel should be discontinued.
Warning 3
In patients with recent transient ischaemic attack or stroke who are at high risk of recurrent ischaemic events, the combination of ASA and clopidogrel has been shown to increase major bleeding. Therefore, such addition should be undertaken with caution outside of clinical situations where the combination has proven to be beneficial.

This medicine is indicated in chronic stable angina (consistent duration and severity of brief episodes of pain, pressure, squeezing and tightness in chest), coronary artery disease (narrowing or blockage of arteries of heart muscles) with normal sinus rhythm (normal heart rate) who have a contraindication or intolerance to beta blockers when monotherapy is inadequate and heart rate is more than 60 beats per minute. It is also used in patients with stable chronic heart failure, with systolic dysfunction (impairment in contraction of ventricles). Patients should be monitored regularly for atrial fibrillation (irregular heart beat), QT syndrome or QT prolongation (a condition in which there is increased risk of irregular heart beat). For intraventricular defect (disturbance in propagation of impulse) and ventricular dyssynchrony (unsynchronized contractions of ventricles) patient should be monitored closely. After any changes in treatment, the patient's blood pressure should be monitored.
